RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

By participating in any class, event, open mat, private lesson, seminar, competition, fitness activity, or other program offered by The Den / She Devil Jiu Jitsu Club ("The Den"), you acknowledge that you have read, understood, and voluntarily agree to the terms of this agreement.

Assumption of Risk

I understand that Brazilian Jiu Jitsu, martial arts, self-defense training, strength and conditioning, and related activities involve inherent risks. These risks include, but are not limited to:

• Cuts, bruises, and abrasions
• Sprains, strains, and fractures
• Joint injuries
• Head injuries
• Neck and spinal injuries
• Illness or communicable diseases
• Permanent disability
• Death

I knowingly and voluntarily assume all risks associated with participation in activities at The Den, whether those risks are known or unknown, foreseeable or unforeseeable.

Release of Liability

In consideration of being allowed to participate in activities at The Den, I, on behalf of myself, my heirs, executors, administrators, personal representatives, assigns, spouse, family members, and anyone acting on my behalf, hereby release, waive, discharge, and covenant not to sue The Den / She Devil Jiu Jitsu Club, its owners, instructors, coaches, employees, volunteers, independent contractors, affiliates, members, and landlords from any and all liability arising from injury, illness, disability, death, property damage, or loss resulting from my participation, including claims arising from the ordinary negligence of any released party.

Indemnification

I agree to defend, indemnify, and hold harmless The Den and all released parties from any claims, lawsuits, damages, costs, expenses, or attorney's fees arising from my participation or from the participation of any minor, guest, or family member for whom I am responsible.

Equipment

I understand that The Den purchases or leases equipment from third-party manufacturers and makes no warranties regarding the condition or performance of such equipment. I voluntarily accept the risks associated with the use of all equipment and facilities.

Health Certification

I certify that I am physically able to participate in martial arts training.

I agree to inform an instructor of any medical condition, injury, pregnancy, physical limitation, or other circumstance that could affect my participation or safety.

I understand that The Den does not provide medical treatment or medical advice.

Hygiene & Safety

I agree to follow all academy rules regarding hygiene, safety, conduct, and respectful behavior.

I understand that instructors may remove me from class or deny participation if they believe I present a safety risk to myself or others.

Photography & Media Release

I authorize The Den / She Devil Jiu Jitsu Club to use photographs and video of me (or my minor child) for promotional purposes, including social media, printed materials, and the academy website.

Acknowledgment

I certify that I have carefully read this entire agreement, understand its contents, understand that I am giving up certain legal rights, including the right to sue, and voluntarily agree to all of its terms.
